The Max Schewitz Foundation takes every precaution to protect our users' information. When users submit sensitive information via the Web site, your information is protected both online and offline. When our registration/order form asks users to enter sensitive information, that information is encrypted and is protected with the best encryption software in the industry. The Max Schewitz Foundation uses Secure Sockets Layer (SSL) technology to keep your personal information as secure as possible. The Max Schewitz Foundation helps to protect your information by working with partners that provide a secure and safe environment for credit card donations.

Privacy Policy
I understand that the Max Schewitz Foundation will endeavor to keep my child’s health information confidential pursuant to the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and its related Rules and Regulations and other state laws. I understand that my student’s results may be used, after being de-identified, for research purposes.

Extra Inspection Protocol
An echocardiogram (echo) uses sound waves to look at the function of the heart. An echo is used to clarify findings on the EKG or as an extra inspection protocol for certain athletes. Students thought to benefit from this additional test, which is provided free on test day, are selected by the cardiologist.

Refund Policy
No refunds will be issued; registration fee will be considered a donation to the Max Schewitz Foundation. This donation will support the education, prevention and research of sudden cardiac death in young people.
